REFUND & RETURN POLICY
At SDS Medical Supplies, Inc., we are committed to providing quality medical equipment and supplies to our patients. Please review the following policy regarding copayments, out-of-pocket payments, returns, and equipment exchanges.
COPAYMENTS & OUT-OF-POCKET PAYMENTS
All copayments, deductibles, coinsurance, and other out-of-pocket payments made by the patient are nonrefundable, except as specifically provided below.
A refund may be issued if SDS Medical Supplies, Inc. is unable to provide the ordered equipment within 180 days of receiving the applicable payment, provided the patient has not received the equipment and no other contractual or payer requirements prevent the refund.
Refunds, when applicable, will be processed using the original payment method whenever reasonably possible.
EQUIPMENT RETURNS
Due to the nature of medical equipment and supplies, all equipment is non-returnable once received by the patient, except as expressly stated in this policy.
Equipment may not be returned simply because the patient changes their mind, no longer wants the equipment, or determines that the equipment is no longer needed.
DEFECTIVE OR MALFUNCTIONING EQUIPMENT
Equipment may be eligible for an exchange if it is determined to have a defect or malfunction.
Any defect or malfunction must be reported to SDS Medical Supplies, Inc. within 24 hours of the equipment being received and/or assembled, whichever is applicable.
The patient should provide:
- The patient’s name and order information;
- A description of the defect or malfunction;
- The date the equipment was received and/or assembled; and
- Photographs or other documentation when requested.
SDS Medical Supplies, Inc. may require the equipment to be inspected or evaluated before approving an exchange.
If the equipment is determined to be defective or malfunctioning and the issue is reported within the required 24-hour period, SDS Medical Supplies, Inc. may provide an exchange for the same or an appropriate replacement item, subject to availability and applicable payer requirements.
PATIENT RESPONSIBILITY
Patients are responsible for promptly inspecting equipment upon delivery and, when applicable, during or immediately after assembly.
Patients should contact SDS Medical Supplies, Inc. as soon as possible if they believe equipment is defective, damaged, or malfunctioning.
